The publicly available links for SCOM management packs don’t always get updated correctly. This has been an issue for the last few releases of the SQL Server MP. As of today, this is the latest release (official download page):
Microsoft System Center Management Pack (Release To Manufacturing) for SQL Server
Version: 7.12.1619.0
NGL, this download offering is a bit sloppy. You’ll notice that all of the individual Windows SQL MPs are listed in addition to the SQLServerMP.Windows.msi. This is atypical and a bit confusing.
If all you need are the Windows packs, just download the applicable .MSI shown below.

Microsoft System Center Management Pack (Release To Manufacturing) for SQL Server is version-agnostic, which means that it supports discovery and monitoring of SQL Server 2016, 2017, 2019, 2022, 2025 and upcoming versions.
With this management pack, you can monitor various SQL Server components, such as Database Engines, Databases, and other SQL-related objects.
Monitoring gives you the insights about such metrics as performance, availability and configuration monitoring, performance and event data collection.
In addition to health monitoring capabilities, this management pack includes dashboards, diagram views, state views, performance views, alert views, and diagnostic tasks that enable near real-time diagnostic and remediation of detected issues.
You can easily integrate monitoring of SQL Server components into your service-oriented monitoring scenarios.
Microsoft System Center Management Pack for SQL Server is intended for Operations Manager 2016, Operations Manager 2019, Operations Manager 2022, and Operations Manager 2025.
